Audit Trail

An audit trail is a chronological record of all actions, changes, and decisions within a system, providing evidence of who did what, when, and why.

An audit trail captures every significant event within a system in chronological order. This includes who created a record, who modified it, what values changed, who made decisions, what communications were sent, and what documents were generated or uploaded.

In regulated industries, audit trails are not optional — they are a compliance requirement. The FCA requires financial firms to maintain records of customer interactions and decisions. The CQC expects healthcare providers to demonstrate consistent processes. GDPR requires organisations to show how personal data was processed.

Effective audit trails are automatic (not relying on users to manually log their actions), immutable (cannot be modified or deleted after the fact), comprehensive (capturing all relevant events, not just a subset), and accessible (easy to search, filter, and export for regulatory review).

SwiftCase's platform automatically generates comprehensive audit trails for every case, workflow execution, document, and communication. Every field change, status update, task completion, and system action is logged with timestamps and user attribution, providing the evidence base that regulated industries require.
